ALL HCS Students

  • A school physical will NOT be required for school entry this upcoming 2020-2021 school year in response to COVID-19. This will help relieve congestion in healthcare offices and prevent unnecessary exposure to our Hope families. 

  • HOWEVER, the NM Dept. of Health still requires students and schools to be up-to-date on immunizations in order to attend school.  Please pay special attention to this if your child is in PreschoolKindergarten or is an upcoming 7th grader.  

  • Please call your physician’s office to inquire as to whether your child is up-to-date on immunizations for school entry.

Magnus for 2020-21

Student Health Forms (Available in Magnus Health the first week of July)
  • Student’s health consent forms and history will need to be reviewed and completed for any changes for the 2020-2021 school year. 

  • In order to ensure that we are doing our very best to meet the health needs of all of our students and keep them as healthy as possible, students whose health information is not complete in Magnus by the first day of school cannot be admitted to school.  

  • You will begin receiving weekly emails around the middle of July indicating that it is time to fill out your forms.  
New Mobile App
  • Great News! Magnus Health has a new, user-friendly app that makes it very easy to complete your student’s forms from your phone or tablet! 

  • Click here for instructions on how to download the app and create a username / password.  This app includes additional benefits that were not previously available when logging in through ParentsWeb on FACTS, such as receiving a notification when your child has a treatment note from a health office visit!  
If you are a new student, you will find Magnus Health located in your ParentsWeb/FACTS log-in. 

Elementary & Mid School Students

  • APIAL will not require a sports physical dated this year in order to participate in Elementary and Middle School sports. 

  • Additional forms will likely be required upon registering your student online at the APIAL website.

High School Students

  • School physicals obtained last year are acceptable for participation in NMAA High School sports as long as your student does not have a new condition or injury AND has NOT tested positive for COVID-19 since the physical that was obtained last year after April 1 of 2019.  

  • NMAA will have a waiver/checklist form in place of a 2020 student physical that will be available to access in Magnus Health.
Please DO NOT complete Magnus health forms until the first week of July.
