All athletes must have a current physical form on file and have paid the $5.00 catastrophic Insurance Fee in order to participate in any sports practices or games.  Click below to download the relevant forms:

• Dual Participation form
• Transportation Release form

Please submit the completed forms with Insurance Fee to the High School Office.
