TRANSCRIPT
INFORMATION FROM THE REGISTRAR’S
OFFICE
FORMS THAT CAN BE DOWNLOADED:
High School Transcript is a copy of a student’s
school records (grades) from grades 9 - 12.
Official transcript is a copy that has the rank,
school seal, date,
and official’s signature, and is sent between
schools or institutions.
Unofficial transcript is a copy that is stamped
UNOFFICIAL on
it and can be given, sent or picked up by parent
or student, if
student is over 18 years of age at time of request.
Official and Unofficial Transcript Release
Form are combined
on one sheet and needs to be signed
by both parent and student, if student is under 18
years of age. This signed form needs
to be in the student’s permanent file
before an official transcript is sent or unofficial transcript is sent out or picked up.
A copy of your student’s unofficial
transcript is recommended for your personal files.
Please check to see that the transcript
is correct, up to date, and has all classes listed
that the student has taken (summer school, transfer course(s), correspondence course(s), etc).
Transcript Request Form can be used to request
where you would like the student’s
transcript(s) to be sent.
SENIOR FINAL REQUEST FORM needs to
be completed with the name of the college or institute
where you would like the final transcript sent (can
request transcript to be sent
to more than one location).
Any overnight or special deliveries must be paid
for in advance.
